KERN COUNTY, Calif. — Omni Family Health announced on Thursday that they have opened two Health Homes Program (HHP) clinics in Kern County.
One of the new clinics is located at 525 Roberts Lane in Bakersfield and the other at 655 S. Central Valley Hwy in Shafter.
“Our goal as a team is to guide our patients in managing chronic conditions, including ensuring they have access to the community resources available to them to address their social determinants of health. By empowering the patient with self-management skills, they can more effectively collaborate with their primary care providers and specialty providers to improve their health,” said Stefanie Medina, Health Homes Program Manager, in a statement released by Omni Family Health.
Omni Family Health is a network of 33 health centers located throughout Kern, Kings, Tulare, and Fresno Counties.
The goal of the HHP clinics is to put physicians, pharmacy, specialists, behavioral and mental health services, and social support services all under one roof.
Omni Family Health partnered with Kern Health Systems on the new clinics.
“To improve the health of our Kern Family Health Care members, it’s necessary to address their physical, behavioral, and social needs. That is the focus of this Program," said Dr. Martha Tasinga, Chief Medical Officer of Kern Health Systems elaborates.
